General Information
All new students are required to submit the following information in their My Student Health Portal accessed from the MyÃÛÌÒµ¼º½ Portal (Available after June 16, 2025):
- Immunization record - Due August 1, 2025
- Medical history - Due August 1, 2025
- Health insurance - Due September 26, 2025
Immunization Record
See the ÃÛÌÒµ¼º½ immunization requirements for all incoming students in 2025-2026. New students are required to provide immunization information online using the My Student Health Portal from the MyÃÛÌÒµ¼º½ Portal.
Medical History Form
New students are required to provide their medical information online using the My Student Health Portal from the MyÃÛÌÒµ¼º½ Portal.
Health Insurance Waiver
Student Health Insurance is required - those with their own coverage must file a waiver each year. Students not completing the waiver form by September 26, 2025, will be automatically enrolled in and charged for the ÃÛÌÒµ¼º½-sponsored health insurance plan. Students can access the waiver by clicking on the Gallagher Student Health Tile in their MyÃÛÌÒµ¼º½ Portal. Students will be able to access the health insurance waiver 4-5 business days after enrolling in 6 units for the fall 2025 quarter. We strongly encourage students to submit the waiver as soon as it is available to avoid late fees and holds on their ÃÛÌÒµ¼º½ student account.
IMPORTANT: The waiver form is required each academic year. Students must be enrolled in 6 units to access the waiver form.

Medical History Form: August 1, 2025
Health Insurance Waiver: September 26, 2025
Note: Enrollment in 6 units required to access these forms.
For students who deposit after August 1 to confirm their enrollment at ÃÛÌÒµ¼º½, the deadline for the Immunization Record and Medical History Form is 10 days after submitting the deposit.
